Description Job Title: Associate Social Worker (License-Eligible Clinician) Department: Behavioral Health Reports to: BH Director FLSA Status: Exempt Job Summary The Associate Social Worker (ASW) will be an integral part of our healthcare team, providing essential mental health services to patients in a community-focused, compassionate manner. As an ASW at UCHC, you will have the opportunity to make a meaningful impact on the lives of individuals and families who may face barriers to accessing quality healthcare. General Accountabilities Provide mental health services to diverse patient caseloads. Monitor, evaluate, and record client progress according to best practices. Interview patient and family members; prepare psychosocial assessments, addressing all pertinent issues of participants. Collaborate with outside agencies to ensure coordinated care for a variety of populations. Evaluate data and work with the care team to develop a single plan of care that supports the patient’s overall health and well-being. Monitor and coordinate the delivery of services for clients as related to behavioral health. Link clients with other treatment providers within the primary care setting and external providers with consent. Function as a member of a multi-disciplinary team. Maintain protocols and procedures for integrated behavioral health activities. Provision of telehealth/remote services as permitted by funding/revenue reimbursement. Ensure charting and documents meet UCHC quality assurance and funding source requirements. Work closely with the Quality Assurance department to support documentation compliance. Perform other tasks as required by management. Requirements Job Qualifications Master of Social Work required. Registration with California Board of Behavioral Sciences required; working toward licensure in good standing. Proof of ACSW registration number required upon hiring. Appropriate certifications and/or licensure with at least three (3) to five (5) years of related experience. Knowledge of evidence-based behavioral assessments and interventions relevant to medical conditions, e.g., disease management, treatment adherence, and lifestyle change. Care-management skills and knowledge of local resources for outside referrals. Consultation liaison skills. Experience working with underserved communities such as undocumented workers, unaccompanied minors, LGBTQ+ populations, communities of color, youth/adolescents, and those experiencing homelessness, substance abuse, and/or mental illness. Employees are responsible for maintaining individual certifications as required by job function or by law and provide verification and recertification when requested by management. Skills Excellent verbal and written communication. Problem solving skills. Active listening. Social perceptiveness. Critical thinking. Ability to utilize Microsoft Office and Google Suite programs. Skills: Language Bilingual – English/Spanish required. Physical Requirement Physical effort which may include occasional light lifting to a 25-pound limit, and some bending, stooping or squatting. Considerable walking may be involved. The ability to sit or stand for extended periods of time is required. Flexibility to work in multiple locations throughout the week.
