Restorative Nurse (LPN)

The Restorative Nurse is responsible for coordinating and overseeing restorative nursing programs for residents throughout the facility. This individual will help bridge communication between nursing and therapy departments while ensuring restorative interventions are properly carried out, documented, and aligned with each resident's care plan and goals. The ideal candidate is organized, compassionate, clinically knowledgeable, and passionate about improving resident outcomes.

Key responsibilities include:

1. Coordinating and overseeing the facility's restorative nursing program
2. Collaborating with the Director of Nursing, therapy team, physicians, and interdisciplinary team members regarding restorative care needs
3. Participating in the MDS/RAI process and care plan development related to restorative nursing programs
4. Identifying residents appropriate for restorative services and initiating therapy referrals/evaluations as needed
5. Monitoring resident progress and updating restorative programs based on changing needs and goals
6. Ensuring restorative nursing documentation is completed accurately and timely
7. Educating residents, families, and staff on restorative nursing interventions and goals
8. Supervising restorative nursing assistants and providing direction to nursing staff related to restorative care
9. Promoting resident independence, mobility, strength, and overall quality of life
10. Ensuring compliance with all federal, state, and facility regulations and policies

Qualifications include:

1. Current Licensed Practical Nurse (LPN) license in good standing
2. Long-term care or skilled nursing experience preferred
3. Knowledge of restorative nursing practices, care planning, and documentation
4. Strong communication, leadership, and organizational skills
5. Ability to work collaboratively with therapy and nursing departments
6. Compassionate, patient-centered approach to resident care
