Physical Therapist (PT)

Join an in-house therapy team where you'll have the autonomy to provide individualized, evidence-based care while building meaningful relationships with residents and families. As a Physical Therapist, you'll evaluate, develop, and implement personalized treatment plans that help residents regain strength, restore function, improve mobility, and maximize independence. Working collaboratively with our interdisciplinary team, you'll play a vital role in helping residents achieve their highest level of function across Skilled Nursing, Assisted Living, and Long-Term Care.

Key Responsibilities

• Perform comprehensive physical therapy evaluations and develop individualized treatment plans based on resident needs and physician orders.
• Deliver evidence-based therapeutic interventions to improve strength, mobility, balance, endurance, pain management, and functional independence.
• Monitor resident progress, reassess outcomes, and modify treatment plans as appropriate.
• Supervise and mentor Physical Therapist Assistants (PTAs) and Rehabilitation Technicians while promoting high-quality clinical care.
• Collaborate with physicians, nursing staff, occupational and speech therapists, and the interdisciplinary team to coordinate resident care.
• Educate residents, families, and caregivers regarding treatment plans, safety techniques, home exercise programs, and discharge recommendations.
• Maintain timely, accurate, and compliant clinical documentation.
• Ensure therapy equipment is maintained in a safe, clean, and functional condition.
• Participate in quality improvement initiatives, infection prevention programs, interdisciplinary meetings, and continuing education.
• Foster a compassionate, resident-centered environment that supports dignity, independence, and exceptional outcomes.

Required Qualifications

• Doctorate or Master's degree in Physical Therapy from an accredited program.
• Current Physical Therapist license in the State of Kentucky.
• Current CPR certification (or ability to obtain within required timeframe).
• Strong knowledge of physical therapy evaluation techniques, treatment interventions, and rehabilitation principles.
• Excellent communication, interpersonal, and organizational skills.
• Ability to work independently while collaborating effectively within an interdisciplinary healthcare team.
• Commitment to compassionate, resident-centered care.
• Ability to supervise Physical Therapist Assistants and support staff as appropriate.
• Ability to meet the physical demands of the position, including lifting up to 50 pounds, prolonged standing and walking, bending, pushing, pulling, transferring residents, and assisting with therapeutic activities.

Preferred Experience

• Previous experience in skilled nursing, long-term care, rehabilitation, or post-acute care.
• Experience treating geriatric residents with orthopedic, neurological, cardiac, and medically complex conditions.
• Knowledge of Medicare Part A, Part B, PDPM, rehabilitation documentation, and electronic medical records (EMR).
• Experience leading or mentoring rehabilitation team members.
• Strong clinical reasoning, time management, and problem-solving skills.
• Passion for helping residents achieve their highest level of function and independence.
