An outstanding candidate will demonstrate an understanding of resident-centered care/service and have experience in geriatric nutrition and therapeutic diets. The primary responsibilities of the Registered Dietician will include but are not limited to: Maintaining knowledge of state and federal regulations as they pertain to nutrition and dining in long-term care settings. Assessing complex nutritional needs and developing appropriate interventions. Conducting comprehensive nutritional assessments for residents upon admission, after any significant change in condition, and regularly as required, developing individualized nutrition plans based on residents' medical conditions, nutritional needs, preferences, goals, and physician orders. Monitor residents for changes in weight, diet, and other nutrition-related risks and recommend appropriate interventions. Participating in an interdisciplinary care planning team and communicating recommendations to residents, families, physicians, and all other members of the care planning team.Why choose Oaknoll?
