What You’ll Do Lead and coordinate individualized hospice care plans Provide skilled nursing care including pain and symptom management Supervise LVNs and CHHAs in the field Collaborate with physicians and the interdisciplinary team Educate and support patients and families during critical moments\ Make a real impact every single day Who You Are Licensed Registered Nurse (RN) - BSN/MSN (preferred) Experienced in hospice, home health, or case management (preferred) Compassionate, independent, and confident in the field Comfortable with EMR/documentation #J-18808-Ljbffr Roze Room Hospice
