General Summary of Position
The Night ED/ICU Pharmacist plays a crucial role in ensuring the safe, precise, and timely review, verification, preparation, and distribution of medications in both adult and pediatric emergency and critical care settings during overnight shifts. This position is primarily located in the ED and operates on a 7 on/7 off schedule .
This pharmacist is accountable for overseeing the assigned ED and ICU responsibilities while also providing support to other night-shift pharmacists based on patient needs, workload, emergencies, and departmental requirements. The role emphasizes adherence to hospital policies, relevant state and federal regulations, and patient safety standards, while delivering comprehensive clinical and operational pharmacy support to the healthcare team.
Key Responsibilities- Administration: Guides clinical staff to ensure medications are administered safely and accurately. Provides advice to ensure medications are given at the correct time, in the prescribed dosage, and via the appropriate route. Communicates any significant adverse reactions or concerns regarding medication administration to patients and clinical staff, and discusses unresolved issues with relevant healthcare providers.
- Departmental Goals: Contributes to achieving departmental objectives and adheres to policies, procedures, quality standards, and safety protocols. Complies with governmental and accreditation regulations.
- Dispensing: Prepares medications safely using proper techniques, including precautions for hazardous medications. Ensures accurate medication preparation and supervises non-licensed staff in this process. Labels medications according to hospital policies and regulations, ensuring safe and accurate administration. Dispenses medications in quantities sufficient for patient needs and in the most ready-to-administer forms.
- Medication Management: Manages recalled, discontinued, unused, expired, or returned medications according to laws and hospital policies. Inspects returned medications for integrity before restocking and segregates outdated or recalled items. Ensures timely administration of first doses for therapeutically monitored medications and documents clinical notes for ongoing monitoring.
- Investigational Medications: Adheres to established policies for the management of investigational medications, including their storage, dispensing, labeling, distribution, and documentation. Reviews investigational protocols before dispensing.
- Monitoring: Responds effectively to actual or potential adverse drug events and medication occurrences. Takes appropriate actions when such events are identified and complies with reporting requirements using the Medication Occurrence Reporting System and other relevant platforms.
- Ordering: Ensures medication orders are complete, accurate, and appropriate for the patient's condition. Verifies documented diagnoses for each medication ordered and reviews orders for appropriateness, including drug dosage, frequency, route, and potential interactions.
- Quality Improvement: Participates in multidisciplinary teams focused on quality and service enhancement and evaluates the hospital's medication management system for risk points, identifying areas for safety improvements.
- Patient Information: Collects and maintains patient-specific information necessary for safe medication management, including age, sex, height, weight, current medications, allergies, diagnoses, and relevant lab values. Engages in information exchange with healthcare professionals.
- Additional Duties: Performs other responsibilities as assigned.
- Selection and Procurement: Ensures that medications for diagnosis or administration are selected and procured based on established criteria. Demonstrates knowledge of the formulary list and adheres to policies regarding non-formulary medications and medication shortages.
- Storage: Ensures medications are stored safely and properly throughout the hospital, adhering to appropriate par levels and stability conditions. Secures medications according to hospital policies and regulations, particularly controlled substances, to prevent diversion.
- Inspection: Regularly inspects medication storage areas in accordance with hospital policies, ensuring emergency medications are available, secure, and stored appropriately.
- Patient-Brought Medications: Follows hospital policy regarding medications brought in by patients or families, determining their usability and evaluating their integrity.
Education
- Bachelor's degree and a valid pharmacist license in the State of Maryland are required.
- An advanced degree is preferred.
Experience
- 1-2 years of clinical hospital pharmacy experience with a solid understanding of pharmaceutical care concepts is required.
Licenses and Certifications
- Pharmacist license in the State of Maryland is required.
- Residency or certification is preferred.
Knowledge, Skills, and Abilities
- Strong verbal and written communication skills.
- Basic computer skills are preferred.
This position offers a salary range of USD $102,606.00 - USD $196,996.00 per year.
