TriWest Healthcare Alliance is seeking a Population Health leader to guide the PH program for the T-5 TRICARE West Region contract. You will collaborate with data management to apply predictive analytics and stratify beneficiaries for Medical Management programs, directing staff and interventions to improve health outcomes and HEDIS scores.
