Minimum Requirements

• Bachelor‘s, Master‘s or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• Home Health experience
• Experience with Matrix EMR

Assignment Details

• 5 days per week with flexible hours
• Productivity expectations: 27–30 points per week
• Territory: West Valley, west of I-17 to Loop 303
• Charting can be completed at home
