Key Responsibilities

• Conduct thorough assessments and develop comprehensive plans for patients experiencing both acute and chronic health issues, such as hypertension, diabetes, and respiratory conditions.
• Utilize advanced diagnostic tools and techniques to order and interpret laboratory and imaging tests, tailoring diagnostic pathways to individual patient needs.
• Prescribe appropriate medications and therapies, closely monitoring their effectiveness and making necessary adjustments to optimize patient outcomes.
• Educate patients and families on health-related topics, empowering them with knowledge on disease prevention, lifestyle modifications, and self-management strategies.
• Document all patient interactions and care plans meticulously, adhering to regulatory standards and organizational policies to maintain accurate medical records.
• Participate in regular quality improvement initiatives, contributing insights and strategies that enhance care quality and operational efficiency.
• Stay abreast of the latest developments in healthcare practices and regulations, integrating new knowledge into patient care approaches.

Patient Population / Client Focus

Your main focus will be on a diverse patient population ranging from young adults to seniors. Qualifications

• Current certification as a Nurse Practitioner in the state of Texas.
• Active and unrestricted nursing license.
• A minimum of a Bachelor's degree in Nursing; Master's degree in Nursing is preferred.
• At least 3 years of clinical experience, preferably in a primary care or a relevant specialty area.
• Exceptional communication and interpersonal skills, with a strong ability to connect with patients and colleagues alike.
• Highly developed analytical and problem-solving skills, able to make informed decisions in fast-paced environments.

Preferred Experience

While 3 years of experience is required, we prefer candidates with advanced practice experience in concierge medicine or house calls, as they can adapt quickly to our unique service delivery model. Familiarity with electronic medical records (EMR) systems and telehealth platforms is highly beneficial.

Compensation and Benefits

The position offers a highly competitive salary range of $120,000 to $220,000 annually, commensurate with experience and qualifications.
