Key Responsibilities

Adheres to the CareStar Rule in performance of job responsibilities.

Understands and complies with CareStar Policies and Procedures.

Maintains confidentiality as related to patient information. Any disclosures of confidential information made unlawfully outside the proper course of duty will be treated as a serious disciplinary offence.

Follows the Acceptable Use Policy while using any information systems owned or controlled by CareStar.

Perform initial and ongoing assessments using approved tools and procedures.

Evaluate functional status, service needs, supports, and risks.
-
Complete all required documentation accurately and on time.
-
Communicate assessment findings and follow-up needs to designated parties.

Coordinate with members, caregivers, case managers, MCOs, and agency staff as needed.

Maintain compliance with policies, confidentiality rules, and reporting expectations.

Participate in training, quality assurance, and calibration activities.
Minimum Qualifications
-
A Bachelor's Degree or a minimum of 4 years of professional experience in the field of intellectual, developmental, and/or physical disabilities required.

Effective organizational, interpersonal, communication and leadership skills.
-
Effective computer skills with familiarity with suite of Microsoft Office programs.
-
Reliable transportation to travel within assigned region as business requires.
-
Valid driver’s license and car insurance as required by State law.
