Job Description We are looking for a Registered Nurse (Case Manager Experience) for our team in Chicago, IL. Salary: 80,000-90,000 Monday-Friday, 8am-5pm No on-call No weekends Additional Details Field work with mileage reimbursement provided. When not in field, will be in the Chicago office doing virtual visits or also working at home (company laptop provided). Job Summary The Registered Nurse (Case Management) role involves a mix of field work and virtual visits. The RN is expected to be self‑motivated and require minimal oversight. Key responsibilities include supporting census management, reviewing patient data, coordinating transitions of care and post‑discharge planning with healthcare providers, as well as potentially conducting outreach to patients by phone. The RN will play a crucial role in new patient engagement and overall care coordination. Qualifications IL RN license. BSN degree. At least 5 years RN experience. At least 3 years of care management experience in the health plan, home health, and/or hospice. Case Management experience on the provider or payor side with a focus on patient interaction.
