Responsibilities The Physical Therapist will primarily provide outpatient rehabilitation services in an in‑home setting for the older adult population. The setting may be a patient’s private residence or a room in an independent living facility. The therapist evaluates and treats in the most functional environment – the patient’s home. Key duties include: Assessing patient needs, developing, and delivering individualized physical therapy programs. Identifying and making referrals to other disciplines to address comprehensive patient needs. Implementing therapy programs designed to restore, reinforce and enhance performance. Instructing, educating and training patients and caregivers in essential skills and functions. Directing activities that promote and maintain health. Managing a flexible schedule and working in various settings. Supervising Physical Therapy Assistants, temporary licensees, students and support personnel as required. Ensuring adherence to departmental and patient treatment schedules, timely clinical and billing documentation, and participation in meetings. Consulting with the rehabilitation team to select appropriate therapeutic regimens. Adapting programs and plans of care to patient needs. Promoting a safe environment and enforcing fire and safety hazards elimination. Ordering, maintaining and reporting on equipment and supplies. Performing screening, examination, diagnosis and care planning, including history, systems review, tests, and interpretation of findings. Developing treatment goals and methods in collaboration with patients and caregivers; implementing the plan and completing documentation. Providing therapeutic exercise, balance, coordination, strength, endurance, functional training, manual therapy, airway clearance, integumentary care, electrotherapy, physical agents, mechanical modalities, health and wellness education, falls assessment and intervention, and case management. Designing, fabricating, fitting and applying adaptive devices such as orthotics and prosthetics. Selecting activities suited to a patient’s physical and mental capacity, intelligence level and interests. Instructing patients in mobility activities and evaluating progress. Conducting routine re‑examinations to modify or progress treatment plans. Incorporating health and wellness programming into patient plans. Performing home and community mobility assessments and providing safe mobility recommendations. Other related duties as required. Qualifications Valid Physical Therapy license in the state(s) of practice or proof of license eligibility. Master’s degree in Physical Therapy, or a Master’s degree in a PT‑related field with a bachelor’s degree in Physical Therapy, or a bachelor’s degree in Physical Therapy plus 5 years of progressively responsible PT experience. PT degree earned outside the U.S. must be evaluated as U.S. equivalent. Valid driver’s license or reliable, independent transportation to patients’ residences.
