Registered Nurse - Hospice

Job Description

Job Summary:
We are seeking a compassionate and skilled RN Care Manager to provide in-home hospice care to patients and their families. The role requires delivering symptom management, crisis intervention, and family education with a focus on comfort and dignity.

Responsibilities:

• Manage comprehensive patient care in the home setting, ensuring symptom relief and comfort.
• Conduct assessments and develop individualized care plans in collaboration with interdisciplinary teams.
• Provide crisis intervention and support to patients and their families during difficult times.
• Educate families on disease progression, symptom management, and care techniques.
• Document clinical activities accurately and maintain detailed patient records.
• Travel within the service area to various patient residences, healthcare facilities, and community agencies.

Qualifications:

• Possess a minimum of 1 year of hospice nursing experience.
• Hold a valid Virginia RN license.
• Demonstrate excellent communication, problem-solving, and decision-making skills.
• Have a valid driver's license, good driving record, and reliable transportation.
• Show ability to build rapport with patients and families and work effectively within an interdisciplinary team.
