How you'll make an impact in this role

• Guide patients through the continuum of care, serving as a primary point of contact and support.
• Act as a liaison between patients, healthcare providers, administration, and external stakeholders.
• Coordinate treatment efforts, support quality improvement initiatives, and contribute to patient education.
• Track outcomes, analyze data for process improvements, and assist with referrals and follow-up care, particularly for underserved populations.

What minimum requirements you'll need

Licensure / Certification / Registration:

• Registered Nurse credentialed from the Illinois Board of Nursing obtained prior to hire date or job transfer date required.
• BLS Provider preferred. American Heart Association or American Red Cross accepted.

Education:

• Diploma from an accredited school/college of nursing OR required professional licensure at time of hire.

What additional preferences we're seeking

• Bachelor's degree in Nursing Preferred
• Bilingual Fluency (English/Spanish) required
• One year clinical case management and/or two years of nursing experience
• Intermediate proficiency with Google Workspace
• Familiarity with Cornerstone (statewide data management information system) a plus
