OT - Home Health Occupational TherapistUnit: Home Health and HospiceUnit Description: Need for an Occupational Therapist for Home Health and Hospice division. 24 to 40 hours per week Provides occupational therapy evaluations, modalities, and treatments. Accurately assesses patient needs; identifies and initiates appropriate therapy interventions. Provides patient and family teaching. Works cooperatively with other patient team personnel in maintaining standards for professional occupational therapy practice and interdisciplinary teamwork.
