We are actively seeking a Physical Therapist (PT) to join a Skilled Nursing Facility in Oklahoma City, Oklahoma for a 13-week contract assignment. This is an excellent opportunity to work with a dedicated rehabilitation team while helping patients restore mobility, improve strength, and maximize their functional independence.

Key Responsibilities:

• Evaluate patients and develop individualized physical therapy treatment plans based on their needs and goals

• Provide evidence-based physical therapy interventions to improve mobility, balance, strength, endurance, and overall function

• Educate patients, families, and caregivers on treatment plans, injury prevention, and home exercise programs

• Monitor patient progress and modify treatment plans as appropriate

• Collaborate with physicians, nursing staff, and interdisciplinary team members to optimize patient outcomes

• Maintain accurate and timely documentation in compliance with facility and regulatory requirements

Requirements

Qualifications:

• Active Physical Therapist (PT) License

• Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program

• Current CPR certification (if required by facility)

• SNF experience preferred but not required

• Strong clinical, communication, and interpersonal skills
