Fountain View is a 99 bed SNF with 16-20 Subacute beds. The remaining beds are short term and long term combined. We also have in-house dialysis for our patients. Responsibilities Title: Physical Therapist Location/work environment: In facility Reporting structure: Reporting to Director of Rehab Assess patient needs, develop recovery plans, and deliver physical therapy. Design programs to restore, reinforce, and enhance physical abilities. Teach patients and caregivers the skills to promote independence and productivity. Qualifications Valid Physical Therapy license in the state(s) of practice, or proof of license and eligibility. Master's degree in Physical Therapy; or Master's degree in a Physical Therapy related field plus a Bachelor's degree in Physical Therapy; or Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience. The qualifying Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent to a degree in Physical Therapy.
