About the Role:

Project Independence Services Inc is looking for a compassionate and dedicated Per Diem LPN to join our team in New Bedford, MA. This is a flexible opportunity for a licensed nursing professional to make a meaningful difference in the lives of our clients while maintaining work-life balance on a per diem schedule.

Responsibilities:

• Provide direct patient care and nursing services in accordance with individualized care plans
• Administer medications and treatments as prescribed by physicians
• Monitor and document patient health status, vital signs, and changes in condition
• Communicate effectively with patients, families, and interdisciplinary care teams
• Assist with assessments and coordinate care needs for clients receiving support services
• Maintain accurate and timely clinical documentation and medical records
• Ensure compliance with state regulations, company policies, and nursing best practices

Requirements:

• Active LPN license in the state of Massachusetts (in good standing)
• Prior experience in home health, community health, or human services preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal skills with diverse patient populations
• Ability to work independently and manage time effectively in a per diem capacity
• CPR/BLS certification required or ability to obtain
• Reliable transportation and ability to travel within the New Bedford, MA area
