You'll help patients and families navigate care options, advance directives, and support services — and complete all documentation electronically right from the patient's home.What You'll DoAssess social and emotional needs related to the patient's healthSupport physicians and clinical team members with psychosocial insightsParticipate in care planning (Plan of Care / POC)Observe, document, and report community resource needsEducate patients and families on available community servicesProvide counseling and guidance to patients and caregiversUtilize and coordinate appropriate community resourcesAssist with discharge planningParticipate in in?service training and educational programsWhat You Need (Qualifications)Master's degree from a Council on Social Work Education–accredited programMinimum 1 year of MSW experienceStrong physical stamina and mental healthPre?employment health clearance (TB test/CXR as required by state)Able to read, follow instructions, and accurately document careSelf?directed; able to work independentlyCurrent CPR certificationProfessional, patient, and flexibleExcellent personal hygiene and communication skillsValid driver's license and insured vehicle per Lorian Health requirementsAble to verbally communicate in the patient's language and document all careWhy Join us?We are committed to our employees' well-being, offering a comprehensive employee benefits package that includes, but is not limited to:· Competitive salary and EAP, 401(K) with 4% match· Paid time off and paid holidays· Mileage reimbursement· Employee referral program and opportunities for growth and advancementThis position is based on a Pay per visit compensation model.Pay per visit ranges from $75 to $90 visit commensurate with experience and depending on type of visit.
