Join Our Growing Home Health Team!Homeside Rehab is seeking compassionate and dedicated Speech-Language Pathologists (SLPs) to provide individualized, evidence-based speech therapy services to patients in their homes throughout the Hudson Valley, New York .

ResponsibilitiesPerform comprehensive speech-language evaluations and develop individualized treatment plans based on each patient's needs and goals.
Provide evidence-based treatment for speech, language, articulation, fluency, voice, cognitive-communication, and swallowing disorders, when applicable.
Deliver skilled speech therapy services in patients' homes while promoting improved communication and functional independence.
Complete accurate, timely, and compliant electronic documentation.
Collaborate with physicians, caregivers, and the interdisciplinary healthcare team to coordinate patient care.
Educate patients and caregivers on home programs and communication strategies.
Communicate effectively with office staff regarding scheduling, patient progress, and clinical needs.
Active New York State Speech-Language Pathologist License (Required)
Minimum 1 year of experience in Home Health, Outpatient, Early Intervention, or Skilled Nursing Facility (SNF) preferred
Excellent clinical assessment and communication skills
Ability to work independently in a home health setting
Basic computer proficiency for electronic documentation
Valid New York State driver's license and reliable transportation preferred
Job Types: Full-time, Part-time, Per Diem
Compensation: Competitive hourly compensation based on experience
