Home Health Occupational Therapist (OT) | San Francisco & San Mateo Counties (One Dedicated Territory per Hire)
We're hiring two Occupational Therapists, one dedicated to San Francisco and one to San Mateo County (Peninsula) , to join our Bay Area teams. Each OT is assigned a single territory, so you can build real familiarity with your patients and your area instead of being stretched across both.

Compensation & Benefits
- Base Salary: $125,000 to $150,000 per year (based on experience)
- Performance Bonus: Up to 25% of base, tied to quality metrics and clinical outcomes
- Total Earning Potential: $156,250 to $187,500 per year

Your Role
- Provide skilled occupational therapy focused on ADLs, adaptive equipment, and functional independence
- Develop, implement, and manage individualized plans of care based on physician orders
- Report changes in patient condition and collaborate with physicians, case managers, and the Director of Nursing
- Maintain accurate, timely clinical records using AI-supported documentation tools
Requirements
- Current, active California Occupational Therapist license (required)
- Minimum 1 year of home health experience strongly preferred
- Valid California driver's license
- Strong organizational skills, clinical judgment, and a compassionate communication style
