Family Nurse Practitioner

Provide clinical and operational leadership for community-based healthcare programs Oversee clinic operations and manage day-to-day operations across multiple clinic locations Supervise Nurse Practitioners and Community Health Administration staff Deliver primary care to underserved and vulnerable populations, including people experiencing homelessness Provide care in mobile, community, and elementary school-based settings Support healthcare services throughout Los Angeles and San Diego Collaborate with interdisciplinary healthcare teams to improve patient outcomes Monitor patient care plans and support continuity of care Oversee clinic workflows, scheduling, and patient-centered service delivery Support billing, credentialing, compliance, and quality improvement initiatives Manage Electronic Health Record systems and operational processes Support grant-funded initiatives and compliance requirements Collaborate with internal and external stakeholders to achieve project goals Assist with federal reporting, outcome tracking, and data collection Support budget monitoring and grant deliverables Partner with community organizations to expand access to care Build relationships with community agencies, schools, and service organizations Represent the clinic at community events and health fairs Support initiatives that increase healthcare access for underserved populations Requirements Master of Science in Nursing (MSN) required Family Nurse Practitioner (FNP) certification required Current and unencumbered Advanced Practice Registered Nurse (APRN) Certification and Registered Nurse (RN) Licensure required Active California furnishing number, National Provider Identifier (NPI), and DEA registration number required Maintain current national and state board certification as a Nurse Practitioner from an approved organization such as ANCC, AANP, or NCC Minimum 5 years of healthcare leadership or management experience Experience serving underserved, vulnerable, or historically marginalized populations Experience managing healthcare programs, clinical teams, or community-based services Pediatric experience required Federal grant management experience preferred Nurse faculty or preceptor experience preferred Experience working with community-based organizations and public health initiatives preferred Experience supporting behavioral health and primary care integration preferred Ability and willingness to travel up to 75% Candidates must reside in San Diego, CA Core Competencies Demonstrates expertise in clinical and operational leadership within community-based healthcare programs, with a strong focus on delivering primary care to underserved populations and managing interdisciplinary teams.
