Registered Nurse – Home Health

Schedule: 8 hours per shift.

Assignment Details: Guaranteed Hours: 40 Contract Weeks: 14

Requirements:

• Current state license as a Registered Nurse - Required
• Homecare Homebase Software Experience - Required
• OASIS Experience - Required
• Proof of current CPR certification - Required
• Current driver's license - Required
• Must have reliable transportation and insurance - Required
• 1-2 years experience as a Registered Nurse in a clinical care setting - Required
• 2+ years Home health experience - Required
• CHPN certification - Preferred
• Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program - Required
• Bachelors of Science in Nursing - Preferred

Responsibilities:

• Assess home health and/or hospice patients in stable and emergent situations to identify physical, psychosocial, and environmental needs.
• Implement, develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
• Educate patients and/or their families regarding care, diet, medications, and home safety.
• Supervise and provide clinical directions to Home Health Aides and nursing staff.
• Monitor assigned cases for compliance with third-party payer requirements.
• Provide effective communication as a liaison to patients/family, team members, and other healthcare professionals.
