Position Summary

Under the supervision of the Medical or Specialty Director, the Nurse Practitioner provides culturally competent, comprehensive healthcare services to patients. The Nurse Practitioner is responsible for the assessment, diagnosis, and treatment of physical and behavioral health conditions, patient counseling and education, preventive care, and ongoing management of patients. This PRN position supports Unity Health Care's commitment to improving health outcomes, enhancing the patient experience, supporting care team well-being, and providing high-quality, safe, patient-centered care.

Key Responsibilities

• Obtain comprehensive patient histories and conduct physical examinations and clinical assessments.
• Determine appropriate diagnostic and screening tests and interpret results.
• Diagnose and treat acute and chronic health conditions within scope of practice.
• Develop and implement individualized treatment plans.
• Provide preventive health services, patient education, and counseling.
• Refer patients to hospitals, specialists, social services, and other healthcare providers as appropriate.
• Provide culturally competent, patient-centered care and utilize translation services when needed.
• Document patient encounters accurately and within established timelines.
• Maintain required clinical privileges, professional licensure, and credentials.
• Participate in quality improvement and patient safety initiatives.
• Conduct peer reviews and provide constructive feedback to colleagues.
• Maintain current clinical knowledge and implement evidence-based best practices.
• Collaborate with health center leadership and staff on patient care, panel management, and population health initiatives.
• Communicate respectfully with patients and actively engage them in their care.
• Respond appropriately to patient concerns and complaints.
• Collaborate with multidisciplinary care teams to provide comprehensive, team-based care.
• Participate in after-hours call rotations as applicable.
• Participate in team, health center, and organization-wide meetings as required.
• Support patient access and provide coverage based on organizational and patient care needs.
• Complete clinical documentation and address outstanding claims within established timelines.
• Serve on committees as requested by the Medical Director or Chief Medical Officer.
• Perform other duties as assigned.

Minimum Qualifications

• Graduate of an accredited Nurse Practitioner program.
• Current Washington, DC professional license.
• Current DC CDS and DEA registrations for prescribing providers.
• Board eligible, board certified, or appropriately licensed in area of specialty.
• Must obtain required board certification or specialty licensure within one year of hire.
• Willingness to participate in on-call rotations as needed.
• Ability to work evenings and weekends when necessary to provide patient coverage.
• Strong clinical judgment and ability to provide safe, comprehensive patient care.
• Ability to work effectively within a multidisciplinary healthcare team.
