Role Overview: A compassionate Physician is needed to deliver high-quality primary care to adults on Medicare in underserved communities. Responsibilities: Provide comprehensive, preventative, and chronic disease care within a team-based model; build strong patient relationships; coordinate multidisciplinary care; and utilize data-driven insights to improve health outcomes. Qualifications & Skills: Strong clinical skills in primary care; ability to collaborate effectively within a team; commitment to patient-centered care; and proficiency in using data to inform treatment decisions.
