Job Overview The Care Manager/Designated Care Manager is responsible for providing high-quality care and services to a consistent group of residents and their families in our assisted living/long-term care and reminiscence neighborhoods. The role includes hands-on care, physical and emotional support, and maintaining a safe, comfortable home-like environment in accordance with federal, state, and Sunrise Senior Living standards. Responsibilities Resident Care Review, document, and initial Daily Log. Assist in developing and updating Individualized Service Plans. Provide direct support for a designated group of residents, including physical checks and symptom documentation. Respond promptly to call bells, security systems, and resident needs. Assist with continence management, hygiene, and ADLs, including IE A (Invite, Encourage, Assist) participation. Maintain resident dignity, safety, and independence. Communicate with families and serve as a resource. Participate in daily cross-over meetings and report changes in resident conditions. Host and engage residents in daily activities and reminiscence programs. Practice standard care precautions for cleanliness, hygiene, and health. Risk Management & General Safety Ensure compliance with occupational health and safety regulations. Follow PPE, fire safety, lock-out/tag-out, and hazardous material protocols. Report accidents, incidents, and unsafe conditions immediately. Maintain clear exits and proper storage of oxygen tanks and other equipment. Housekeeping & Laundry Services Clean resident rooms, common areas, and dining facilities. Wash resident laundry, linens, and napkins as needed. Maintain tidy enforcement of common areas at all times. Dining Service Serve meals, set up and clean dining rooms. Follow resident diet plans and dietary restrictions. Observe and document dining habits and changes. Provide room service delivery when required. Adhere to safe food handling and universal precautions. Reminiscence Neighborhood Responsibilities Invite, encourage, and assist residents in life‑skill and enrichment activities. Integrate “Creating Pleasant Days” into daily routines. Incorporate multi-sensory experiences. Contribute to monthly letters home and letter‑writing activities. Training & Team Success Participate in team meetings, training modules, and engagement programs. Maintain infection control compliance and required training. Perform other duties as assigned. Core Competencies Remain calm in difficult situations. Handle multiple priorities and time‑management. Excellent communication and judgment skills. Problem-solving and decision-making ability. Experience & Qualifications High School diploma/GED (required by state/provincial regulations). CPR Certificate and First Aid (required). Minimum 18 years of age. Preferred: previous experience with seniors. Desire to serve and care for seniors. Ability to make decisions in the resident’s best interest.
