Discharge Planner Job DescriptionA Discharge Planner plays a vital role in the patient care continuum within healthcare settings. The primary objective is to coordinate and manage the safe and efficient discharge of patients from hospitals to their next phase of care, which may include home care, rehabilitation, or long-term care facilities. Working closely with medical staff, patients, and their families, the Discharge Planner ensures that each patient's medical and psychosocial needs are met post-discharge. The position requires a fine balance of clinical knowledge, coordination skills, and compassionate communication.Discharge Planner Responsibilities & DutiesCoordinate and manage patient discharge planning activitiesAssess patients' needs and develop discharge plans for post-hospital careCollaborate with healthcare team members including physicians, nurses, and social workersArrange for home healthcare services or equipment as requiredProvide patients and families with information on discharge plans and available resourcesEnsure all necessary paperwork and documentation are completed accuratelyFacilitate patient referrals to external healthcare facilities or community servicesMonitor patient progress and modify discharge plans as necessaryEducate patients and families on follow-up care and medication instructionsAdvocate for patient needs and preferences throughout the discharge processDischarge Planner Qualifications & SkillsBachelor's degree in Nursing, Social Work, or related healthcare fieldCertified case manager credential (CCM)Previous experience in discharge planning or case managementFamiliarity with electronic health records (EHR) systemsStrong understanding of healthcare regulations and compliance requirementsExcellent communication and interpersonal skillsAbility to work effectively in a multidisciplinary team environmentProficiency in problem-solving and critical thinkingKnowledge of community health resources and servicesFluent in multiple languagesRegistered Nurse (RN), Occupational Therapist Assistant (COTA), Physical Therapist Assistant (PTA) or Licensed Social Worker (LSW) licenseMinimum of 2 years of clinical experience in a hospital or healthcare settingStrong organizational and time-management skillsAbility to handle high-stress situations with calm and efficiencyEffective written and verbal communication abilitiesCompassionate and empathetic patient care skillsBasic computer skills and proficiency with Microsoft OfficeStrong attention to detailAbility to prioritize tasks and manage multiple projects simultaneouslyWillingness to continue professional development and education
