About the Position

Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Monday-Friday Hours per Shift: 8:00 AM - 5:30 PM / 8:00 AM - 5:00 PM Experience: Home Health experience required; strong Med Surg experience considered with some Home Health experience License: Valid nursing license required Certifications: Active BLS required or willingness to obtain Must-Have: COVID and FLU vaccination required; declinations will not be approved. Description: Prescribes, coordinates, and delivers care through the nursing process. Maintains the standards of professional nursing. Supervises Certified Nursing Assistants and coordinates care with other health care workers as assigned. Provides comprehensive initial assessments to all clients and continuously assesses physical, psychosocial, environmental, and community resources available. Utilizes principles of growth and development over the lifespan. Initiates Plan of Care, re-evaluates the need for care, and modifies Plan of Care as ordered or needed. Establishes discharge plans with patients, physicians, other disciplines, and caregivers. Completes OASIS data. Provides patient care using current federal and state regulations. Assesses educational needs of patients, families, and caregivers, and implements appropriate teaching strategies. Completes documentation according to regulatory guidelines. Promotes a clean and safe environment of care using SAFE error prevention habits. Provides the highest standard of privacy and confidentiality in matters involving patients and coworkers by abiding by the Standards of Conduct.
