The role of the Licensed Practical Nurse reports to the Nurse Supervisor. This position, under direct supervision of a registered nurse, is responsible for providing direct patient within the scope of practice. This position will be responsible for completing home visits and ensuring all related documentation is completed in a timely manner.

Essential Position Responsibilities

Chart observations of the patient's condition at each visit
Complete evaluation tasks, including reviewing medication and vital signs
Complete written reports on patients in a timely manner
Provide thorough education for patients and families on proper home health care procedures and strategies
Partner and coordinate care with members of the patient's care team
Partner with registered nurse regarding patients condition or changes in medication

Qualifications

Education: Must be a graduate of an accredited nursing program
Licensure and Certifications: Licensed practical nurse and CPR required. Valid drivers license and auto liability insurance, required.
Experience: Minimum of 1 year experience in a clinical nursing setting, required.
