Key Responsibilities:

• Provide care and services in accordance with individualized plans of care
• Teach patients and families exercises and self-care techniques
• Coordinate and collaborate with other healthcare team members
• Document care accurately and in a timely manner
• Maintain patient confidentiality and follow HIPAA regulations
• Attend continuing education and departmental training
• Promote a positive image of NWVNA through excellent patient care

Qualifications:

• Current PT or PTA license from the Massachusetts Board of Allied Health
• Minimum 2 years experience providing direct patient care in a community or clinical setting
• Associate’s degree and graduation from an accredited Physical Therapy program
• Current valid driver’s license
• Strong communication skills and ability to work independently and as part of a team
• Comfortable using computer technology for documentation and communication
