Gap in coverage- current locums provider not extending after August Location- Both Inpatient outpatient settings located 5 minutes from each other Both Clinic Hospital privileges needed Rounding Admissions Yes Demographics- Adults Avg. patients per day 12-14 Required Procedures- N/A Clinical Activity past 24months- Applicant must provide documentation case list of provision of clinical services 100 patients representative of the scope and complexity of the privileges requested during the previous 24 months. Case list must include diagnosis or procedure and date and facility where performed.
