Nurse Practitioner

Role Overview A Nurse Practitioner at Oak Street Health provides effective and equitable value–based primary care to adults on Medicare, seeing patients independently and collaborating with physicians and a multidisciplinary team. The role focuses on primary care, care coordination, and population health within outpatient practices. Core Responsibilities Provision of exceptional primary care and conduct office visits for routine and acute issues. Administer Annual Wellness Visits and health risk assessments with holistic documentation. Care coordination with other providers, specialists, testing facilities, and agencies. Population health leadership, ensuring eligible patients receive evidence‑based screenings (e.g., breast cancer screening for eligible females every 2 years). Assist the care team with phone triage and outreach. Educate patients on their health conditions, care plans, and treatments. Participate in Oak Street Health promotional activities. Conduct home visits as needed. Other duties as assigned. Required Qualifications Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate. National certification in at least one of the following specialties: Family Nurse Practitioner Adult‑Gerontology Primary Care Nurse Practitioner Adult Nurse Practitioner Gerontological Nurse Practitioner Active, non‑probationary state Nurse Practitioner license. Active DEA license. U.S. Work Authorization. Preferred Qualifications Experience in primary care: internal medicine, geriatrics, or family medicine. Knowledge of HCC documentation, ICD‑10 coding, and Health Risk Assessments. Passion for teamwork and cross‑functional collaboration. Desire to be part of an innovative population‑health model. Bilingual proficiency in applicable areas. Work Schedule Anticipated weekly hours: 40 hours. Time type: Full time. Pay Range Typical pay range: $78,331.00 – $168,714.00 for the position. Final offer depends on experience, education, geography, and other factors.
