POSITION SUMMARY

The Registered Nurse (RN) provides nursing care and services to clients in their place of residence/facility in accordance with the agency’s policies and procedures and under the supervision of a Registered Professional Nurse. QUALIFICATIONS OF THE REGISTERED NURSE (RN)

Graduate of an approved RN NYS Licensed and currently registered as a Registered Nurse MUST have previous Home Health or Long-term Care experience MUST hold a current CPR Certification Ability to read and interpret documents such as safety rules, operating and maintenance instructions and procedure manuals. Ability to perform mathematical operations as related to medication calculations, units of American money and weight measurements, volume, and distance. Good organizational writing, communication, and interpersonal skills. Ability to apply common sense understanding to carry out instructions furnished in written, oral or diagram form. Ability to deal with problems involving several variables in standardized situations. Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

EQUIPMENT OPERATION OF THE REGISTERED NURSE (RN)

Durable medical equipment and nursing equipment. CLINICAL SERVICES OF THE REGISTERED NURSE (RN) OR LICENSED PRACTICAL NURSE (LPN) Follow a designated care plan in accordance with patients needs Perform assigned duties. Including administration of medication, wound care, treatments, and procedures Monitor clients’ conditions; reporting changes to our RN Provides care to clients in accordance with NYS Practice Act Implements medical orders and established plans of care Follows agency policy and procedures Administers medications (oral, IM, SQ, Topical) Performs wound care/dressing adhering to sterile technique Obtains information from the client regarding complaints, status of illness, problems obtaining services from other sources, problems with supplies and equipment, communication to the DHC as appropriate Performs medication review and accountability Performs catheter care Performs enteral feeding Performs ostomy care Assists the physician and/or Registered Professional Nurse in performing specialized procedures Evaluates the appropriateness, cleanliness, and safety of durable medical equipment, supplies, and or appliances. Assists the client in learning appropriate self-care techniques May perform aide supervision, as directed Documents visits, notes and progress notes to reflect services/client activities Communicates with other members of the health care team
