Develop and implement PT plans of care focusing on mobility, strength, balance, gait, endurance, and activities of daily living. Deliver one-on-one and group therapy sessions, monitor progress, and adjust treatment plans as needed. Document evaluations, treatment notes, progress, and discharge planning in the electronic medical record per facility policy and regulatory guidelines. Collaborate with a multidisciplinary team (nursing, OT, SLP, physicians, therapy support staff) to coordinate care and ensure safe, integrated rehabilitation. Requirements Active Physical Therapist license in Washington or eligibility to obtain. Doctor of Physical Therapy (DPT) degree or PT equivalent. 1+ year of clinical experience in skilled nursing facility or geriatric rehabilitation preferred. Ability to work effectively in a skilled nursing facility setting and collaborate within a multidisciplinary team. Strong communication skills and commitment to patient safety and quality of care.
