Physical Therapist

Overview Title: Physical Therapist Location: Lebanon, NH Type: Onsite Local Contract Shift: 08:00 AM – 04:30 PM Length of assignment: 13 Weeks Compensation: $63.00 – $63.30 per hour; $2,520.00 – $2,532.00 weekly base pay 8 hours/day, 5 days/week. The Physical Therapist will primarily provide outpatient rehabilitation services in an in-home setting for the older adult population. Services may be provided in a patient's private residence or in an independent living facility room. The goal is to evaluate and treat in the patient's home and promote independence and functional performance. Responsibilities / Accountabilities Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel in accordance with state licensure requirements and professional standards as necessary or applicable. Adhere to department and patient treatment schedules; complete timely clinical and billing documentation; participate in work-related meetings. Attend and contribute to patient care conferences and related meetings. Refer patients to other disciplines and agencies to address comprehensive needs. Consult with rehabilitation team members to select the most appropriate therapeutic regimen for each patient. Adapt the plan of care according to individual patient needs. Promote a safe environment and address fire and safety hazards; Order supplies and equipment as necessary; maintain and report equipment needs or repairs. Provide patient- and family-centered service with high-quality care and compassionate support. Screen, examine, and evaluate patients (history, systems review, tests and measures) to establish diagnoses, identify impairments, determine expected improvement, and design and plan care; develop goals with patient and caregivers; implement treatment plan and document services. Deliver therapies including but not limited to: Therapeutic exercise (conditioning, balance, gait, neuromotor development, flexibility, strength, endurance) Functional training Manual therapy Airway clearance techniques Integumentary repair and protection Electrotherapeutic, physical agent, and mechanical modalities Health and wellness Chronic disease management education Falls assessment and interventions Case management in home and community settings Design, fabricate, fit and apply adaptive devices, including orthotics and prosthetics, and train patients in their use. Select appropriate activities aligned with the patient's abilities and interests; instruct in mobility skills and assess progress. Perform routine reexaminations to modify or progress the plan of treatment. Incorporate health and wellness programming into treatment plans; conduct home and community mobility assessments with safety recommendations. Perform other related duties as required. Educational / Vocational Requirements Valid Physical Therapy license in the practicing state(s) or proof of license eligibility Master's degree in Physical Therapy, or Master's in a related field plus a Bachelor's in Physical Therapy; or Bachelor's degree in Physical Therapy with 5 years of progressive PT experience Physical Therapy degree earned outside the U.S. must be evaluated for U.S. equivalence Valid driver's license or reliable transportation to patient residences Job Skills Good organizational and time management skills Good verbal and written communication skills Compliance Complies with applicable legal requirements, standards, policies and procedures including the Compliance and Ethics Program, Standard/Code of Conduct, Federal False Claims Act and HIPAA Participates in required orientation and training programs Reports concerns related to non-compliance to supervisor or Compliance Officer Cooperates with monitoring and audit functions and investigations Participates in quality assurance and process improvement activities #J-18808-Ljbffr Creative Solutions Services, LLC
