Nurse PractitionerThe Nurse Practitioner applies advanced education and clinical competencies to achieve patient outcomes. The Nurse Practitioner works on problems of diverse scope and complexity ranging from moderate to substantial.The Integrated Home Care Program is a creative initiative dedicated to transforming healthcare delivery for high-risk patients in the comfort of their own homes. We aim to reduce avoidable hospitalizations and readmissions by providing comprehensive, patient-centered care to individuals with medical complexity and functional limitations. Our team delivers proactive, coordinated care directly where our patients need it most.Main Responsibilities:Conduct comprehensive in-home evaluations and examinationsUse both in-person and telemedicine capabilities to deliver careDevelop and implement personalized treatment plansManage acute clinical issues in the homeMaintain accurate documentationParticipate in medical team meetingsYou will report to our Chief Medical Officer for San Antonio, Texas. As part of this role, you will work onsite visiting patient's homes in San Antonio, Texas.If selected for this role, we will require you to undergo a TB screening test.Required qualifications:Nurse Practitioner or Physician Assistant with 1+ years of experience in Home Health, or a related field.Preferred qualifications:Bilingual in Spanish and EnglishAdditional Information:Language Proficiency Testing: any associate who speaks with a member in a language other than English must take a language proficiency assessment, provided by an outside vendor, to ensure competency. Applicants will be required to take the Interagency Language Rating (ILR) test as provided by the Federal Government.Scheduled Weekly Hours: 40Pay Range: $123,800 - $170,300 per yearThis job is eligible for a bonus incentive plan.
