Care Coordinator - Social Services

Assignment Duration

This is a 13-week contract position.

Schedule

Day Shift: 5x8-Hour (08:00 AM - 04:30 PM) with no mandatory weekends, but opportunities to work weekends available.

Job Description

We are seeking a dedicated Care Coordinator to join our Social Services team. The ideal candidate will provide care coordination, discharge planning, and assist in disease management for adult and geriatric patients. This role involves working closely with the healthcare team to ensure effective communication and continuity of care.

Responsibilities

• Coordinate care for patients across their healthcare journey.
• Develop and implement patient care plans.
• Facilitate discharge planning and follow-up care.
• Manage disease management programs.
• Work collaboratively with interdisciplinary teams.

Required Experience / Certifications / Licensure

• 1 year of experience in care coordination or social services.
• Certification in Basic Life Support (BLS) is required.
• Licensed Social Worker (LSW) certification is preferred.
• Experience with Epic charting system is preferred.
