Social Care Coordinator (HYBRID)

Social Care CoordinatorThis position will be hybrid and will require some on site work. No relocation, provided.Applicants must be authorized to work for any employer in the U.S. Care Compass is unable to sponsor employment visas for this role.ResponsibilitiesThe responsibilities of the Social Care Coordinator will include the following:Essential Functions:Review daily off platform screenings/self-screenings and connecting those with unmet needs to Navigators, responding to daily voicemails/emails that come in from Members or Providers.Provide basic technical support such as looking up information in ePACES and similar platforms, ensuring timely and effective assistance when technical difficulties are encountered.Extract data from Medicaid disenrollment file and disable insurance in UniteUs platform.Monitor and respond to communication(s) received from Members made via phone calls or from the Care Compass Collaborative website.Maintain and manage ticketing systems to track inquiries and ensure timely responses.Act as a liaison between the Social Care Network team and other departments, facilitating clear communication and collaboration.Assist Social Care Navigators with low level screenings and eligibility assessments, as needed.May assume additional responsibilities, as required.Must maintain HIPAA compliance at all times.Minimum RequirementsPossession of a high school diploma or equivalent.Familiarity with the U.S. healthcare system and federally funded healthcare programs.Possesses strong communication skills (written and verbal)Proficient in planning & time managementAbility to work independently with minimal supervisionUtilizes critical thinking skillsPossesses adaptability skills (both in attitude and action)Must have strong personal computer program skills including Microsoft Outlook, Microsoft Office (Word, Excel PowerPoint etc.)Facilitation of meetings either in person or via virtual platforms such as Zoom
