What You’ll Do

• Provide comprehensive primary care services, including assessment, diagnosis, and treatment
• Manage acute and chronic conditions across a diverse patient population
• Perform physical exams, preventative care visits, and health screenings
• Develop, implement, and adjust individualized care plans based on patient needs
• Educate patients and families on conditions, medications, and wellness strategies
• Collaborate closely with physicians and the care team to ensure continuity of care
• Accurately document patient care and maintain timely medical records
• Participate in quality improvement initiatives and help enhance care delivery

Qualifications and Experience

• Masters Degree in Nursing (Required)
• Nurse Practitioners Certification (NP-C) (Required)
• BCLS - Basic Life Support (Required)
• Department Specific Certifications (Preferred)
• Experience in primary care (Preferred)
• 3 years’ experience in supervision and independent decision making in major nursing areas. (Preferred)
