Seeking a dedicated Physical Therapist (PT) to provide high-quality rehabilitative care and therapeutic services aimed at improving patient mobility, strength, balance, functional independence, and overall quality of life. The Physical Therapist will evaluate patients, develop individualized treatment plans, and collaborate with an interdisciplinary healthcare team to achieve optimal patient outcomes.

GUARANTEED HOURS & PAY

• Guaranteed Hours: 35 Hours Per Week
• Pay Rate: $51.00 - $59.50 per hour

REQUIREMENTS

• Active Physical Therapist (PT) License in good standing
• Graduate of an accredited Physical Therapy program
• Current CPR certification preferred
• Strong assessment, treatment planning, and documentation skills
• Excellent communication and patient education abilities
• Ability to work effectively within an interdisciplinary healthcare team
• Previous rehabilitation, skilled nursing, outpatient, home health, or acute care experience preferred

KEY RESPONSIBILITIES

• Perform comprehensive physical therapy evaluations and assessments
• Develop and implement individualized treatment plans based on patient goals and clinical needs
• Provide therapeutic exercises, gait training, balance training, mobility interventions, and functional rehabilitation services
• Monitor patient progress and modify treatment plans as appropriate
• Educate patients and caregivers on treatment plans, exercises, and injury prevention strategies
• Maintain accurate and timely clinical documentation
• Collaborate with physicians, nurses, therapists, and interdisciplinary healthcare professionals
• Participate in care planning and discharge planning activities
• Recommend adaptive equipment and mobility aids as needed
• Promote evidence-based practice and patient-centered care
