Title: Physical Therapist Duration: 3 months contract + Possibility of Extension Location: New Braunfels, TX Shift: 8 AM - 4:30 PM Coverage Areas: 60 mile radius About the Rol eWe are seeking a skilled and compassionate Travel Physical Therapist to deliver high-quality care to patients in a home health setting. In this field-based role, you will provide individualized therapy services to help patients restore mobility, improve function, and enhance quality of life—right in the comfort of their homes. This position is ideal for clinicians who are independent, adaptable, and passionate about patient-centered care . Roles & Responsibiliti es:Provide skilled physical therapy services to patients in their homes in accordance with physician orders and care pl ansEvaluate patient conditions, develop treatment plans, and set functional go alsImplement therapeutic interventions to improve mobility, strength, balance, and overall funct ionEducate patients and caregivers on exercises, safety practices, and ongoing care pl ansMonitor patient progress and adjust treatment plans as necess aryDocument all patient care activities accurately and in a timely man nerCollaborate with interdisciplinary care teams to ensure coordinated and effective treatm entMaintain compliance with clinical guidelines, agency policies, and regulatory standa rdsTravel to patient homes within assigned coverage areas (up to a 60-mile radi us) Minimum Qualificati ons:Active Physical Therapist (PT) lic enseBachelor’s degree (or higher) in Physical The rapyAbility to deliver skilled care independently in a home health set tingStrong clinical judgment and patient assessment sk ills Preferred Qualificat ions:Previous home health exper ienceExperience with HCHB (Homecare Homebase) or similar EMR sy stemsStrong time management and organizational s killsExcellent communication and patient education abil itiesFlexibility and adaptability in a field-based enviro nment Work Environment & Require ments:Field-based role requiring travel to patient homesAbility to perform physical therapy interventions, including patient mobility assi stanceReliable transportation required for travel within assigned radius
