Transfer Team Nurse - Registered Nurse

Job Description BJC West – Saint Luke's Health System in Kansas City, MO is seeking a Registered Nurse to join the Central Patient Logistics Transfer Team. This RN position is self-directed, functioning independently to facilitate internal and external physician access to SLHS. The RN will provide tele‑nursing care to a diverse population of critically ill patients across the Saint Luke's Health System and serve as a resource to colleagues at the unit or department levels. Shift Details Mid‑Shift 11:00am – 11:00pm, self‑scheduling. Responsibilities Function independently to facilitate physician access to the transfer team. Interpret data and prioritize tasks using strong problem‑solving and critical‑thinking skills. Communicate clinical issues promptly via proper system channels. Serve as a resource to colleagues at unit or department levels. Provide tele‑nursing care to critically ill patients across SLHS. Delegate tasks as appropriate and assume preceptor responsibilities when needed. Act as a tele‑health nurse evaluating, triaging, and communicating with SLHS physicians, clinical staff, Care Integration and other clinical and social support for patients discharged from SLHS facilities.
