Home Health Registered Nurse (RN)
Location: Plymouth County, Iowa
Assignment Duration: This is a 13-week contract position.
Schedule: Day shifts (Monday to Friday) with a call rotation of one in four nights, weekends, and holidays.
Job Description: We are seeking a qualified Home Health RN to provide compassionate care to patients in their homes. The ideal candidate will have a strong background in conducting OASIS assessments and developing care plans tailored to individual patient needs.
Responsibilities:
- Conduct in-home assessments, including OASIS evaluations.
- Develop and implement individualized care plans for patients.
- Provide education and support to patients and their families.
- Document patient progress and adjust care plans accordingly.
- Participate in call rotation, responding to urgent patient needs.
Required Experience / Certifications / Licensure:
- Must have at least 2 years of experience in home health care, specifically performing OASIS assessments.
- Previous experience with Epic electronic health record system is a plus.
- Current RN licensure in the state of Iowa.
