Position Title Licensed Practical Nurse for Personal Care Management Program Location Barnstable, MA 02630, USA Boston, MA 02113, USA Cambridge, MA 02139, USA Salary $31 per hour (Full time, Monday‑Friday, 8:30am‑4:30pm core hours) Hours & Orientation Full time. Requires 2‑day orientation in person at Stoughton office. Mileage reimbursement available. Job Description Licensed Practical Nurse (LPN) responsible for performing annual reevaluations of members in the MassHealth Personal Care Services program and other related duties to support the Personal Care Attendant Evaluation Department. Essential Functions Perform annual reevaluations of members in accordance with MassHealth regulations 130 CMR 422.422(C). Submit completed evaluations within specified timeframes. Respond to hours adjustment requests in a timely manner. Respond to MassHealth inquiries and offer professional input regarding PCA evaluation as requested. Learn and adhere to PCA Program standards. Schedule reevaluation visits and update calendar accordingly. Document progress notes in the database in a timely manner. Communicate with PCA Evaluation Department Assistants regarding no‑shows, lack of contact, hospitalizations, termination requests, and other concerns. Submit monthly mileage sheets within the required timeframe. Work collaboratively with other members of the PCA Program. Communicate with Supervisor and office staff when issues arise. Interact with Tempus Unlimited, Inc. offices to resolve concerns related to evaluations and support consumer independence. Attend monthly staff meetings. Report suspicion of fraud to the PCM Management Team. Report all suspected abuse or neglect to the appropriate agency. Represent the agency in a professional, positive manner and provide culturally sensitive services. Perform other duties as assigned by the Manager. Preferred Experience Current Licensed Practical Nurse to practice in Massachusetts in good standing with the Board. At least 2 years recent experience in the direct care of the elderly or disabled population. Fluency in other languages preferred. Familiarity with MassHealth PCA Program preferred. Work Environment Professional office and home office setting; routine use of computer, phone, photocopier, and electronic filing systems. Physical Demands Requires frequent sitting, standing, bending, stooping, walking, carrying, and lifting objects. Travel Travel within coverage area required; reliable transportation necessary for community visits. Other Duties Activities may change at any time with or without notice.
