Active Ohio Physical Therapy license required prior to start of assignment. Current CPR certification. Valid driver’s license, proof of vehicle insurance, and access to a dependable vehicle for patient visits across the service area. Experience providing home health physical therapy preferred. Ability to complete patient evaluations, develop plans of care, and document per CMS and agency requirements. Schedule flexibility to participate in weekend rotation and potential holiday availability as needed. Assignment Details Facility type: Home Health. Shift name: Other. Must be willing to visit patients across the agency’s service area and to float to other nearby agency locations as needed.
