Responsibilities

• Provide physical therapy assistant services under the supervision of a licensed physical therapist in an LTAC/SNF environment.
• Implement and adjust therapeutic exercise programs, gait training, range of motion exercises, strengthening, and modalities as prescribed.
• Monitor patient progress, document treatment response, and communicate changes to the supervising PT.
• Ensure patient safety during mobility and daily activities; support discharge planning and home exercise instruction.
• Maintain accurate, timely documentation in the patient record and participate in chart reviews as needed.
• Collaborate with the PT, nursing staff, and therapy colleagues to coordinate care and meet patient goals; adhere to infection control and safety policies.

Requirements

• Active California Physical Therapy Assistant license or eligibility to obtain licensure.
• Associate degree in Physical Therapy Assisting.
• Experience in LTAC or skilled nursing facility settings preferred.
• Strong communication and teamwork skills; ability to document clearly and work within a team-based model.
• Ability to work in a rotating schedule and travel as assigned.
