Schedule: 8:00 am to 4:00 pm
Coverage Areas: Adams, Allen, DeKalb, Huntington, Kosciusko, LaGrange, Noble Steuben, Wells and Whitley Counties
Location: Fort Wayne IN.
Job Type: Contract
Contract Length: 3 months with possibility of extension
Pay Range: $52/hr to $56/hr
*Pay offered is based on experience, expertise, credentialing, and education.


Duties:

• Perform Start of Care (SOC) visits, comprehensive evaluations, and ongoing assessments for home health patients.
• Develop and implement individualized physical therapy treatment plans based on patient needs and goals.
• Provide skilled physical therapy interventions to improve mobility, strength, balance, safety, and functional independence.
• Educate patients and caregivers on exercises, safety techniques, and home programs.
