Home Health Physical Therapist (PT) Full-Time

Physical TherapistAssess each patient's functional status, muscle strength, mobility, gait, ROM, and rehab potential, including the home environmentCarry out the physical therapy plan of care set by the physicianInstruct patients and families on therapeutic appliances and prosthetic devicesDocument care accurately and on time, including clinical and progress summariesCoordinate with physicians and report patient response to treatment or changes in conditionIdentify needs for other home health services and refer as appropriateParticipate in discharge planning and case conferencesProvide consultation to families/caregivers and inservice education to organization personnelSupervise physical therapy assistants per organization policy and state regulationsCheck in on home health aide care every two weeks when therapy is the only skilled serviceTake on other duties as assignedQualificationsPossesses a degree from a baccalaureate or Master's program in physical therapy approved by an accredited organization.Licensed to practice as physical therapist within the state, if applicable.Two years of appropriate experience as a physical therapist.
