Role: Care Connections Nurse is part of a team designed to effectively assist patients in transitioning from hospital to community. Responsibilities Work with the patient to focus on collaboration between hospital and community‑based providers and arrange post‑discharge services. Function as a member of a multidisciplinary team with the goal of improving the patient’s health status and reducing the need for hospital care. Assist with program development, evaluation, professional education, or supervision of students. Qualifications BSN Degree required within 3 years of start of position. 1 year Basic Life Support – American Heart Association or Red Cross, Registered Nurse – Various.
