Licensed Practical Nurse - LPN Visit Nurse

Compensation: $30 - $38 per visit. Benefits: Full Benefits Package. Location: Twin Cities Metro Area. Schedule: Full-Time Availability.

Position Summary: Home Care is in high demand and we are looking for experienced and caring LPNs to join our team at Home Health Care Incorporated! Successful applicants will have a proven track record as an LPN, with home care experience a huge benefit but not required. We encourage you to apply today!

Responsibilities: Provide patient care according to client's care plan and needs. Perform wound care treatments and assist clients with medication management. Teach the client and/or family members about care and support skills. Communicate regularly with RN Case Managers, Supervisors, and the Care Team.

Qualifications: Licensed as a Practical Nurse / LPN in the state of Minnesota. Driving is required; must have a reliable vehicle with current insurance and a valid Driver's License. Demonstrated ability to work autonomously. Prior Nursing experience in a TCU, Hospice, or Home Health setting. Experience with Electronic Medical Records (EMR) software is preferred.
